The concept: 3D obturation

The concept: 3D obturation

41 minutes

An important factor, often neglected, in the process of root canal treatment is appropriate obturation. So, what are the prerequisites that one can rely upon to obtain a successful, densely filled homogenous inert material in the root canal system? Can we identify the shortfalls before beginning the obturation process?

The video covers

  • Why 3D endodontics? -The requirement of thinking “3D”
  • The concept -3D endo
  • The prerequisite: 3D obturation-centric irrigation

A 3-point strategy for 3D preparation of canals and the vital link between irrigation and obturation has been discussed to lay the foundation for a successful obturation process.

Aims and objectives of obturation

Aims and objectives of obturation

13 minutes

This video defines - what is it that we want to achieve during and after the process of obturation. How do you clinically appreciate and recognise ‘the right time to obturate’ and also lists the clinical situations where a clinician should avoid obturating in the same appointment

Material science

Material science

24 minutes

This part deals with the different materials and overall material evolution for obturation of the root canal systems. The following topics have been discussed

  • The concept of monoblock
  • The reaction of sealer with root dentine
  • Types of gutta-percha
  • Root canal sealers

Preparation for the procedure

Preparation for the procedure

36 minutes

The step-by-step protocol needs to be followed before the actual process of obturation.

The video provides practical pointers for the following

  • Removing the ICM
  • Cone fit
  • GP adjustment
  • GP sterilization
  • Drying the canals
  • Sealer placement technique

Obturation techniques

Obturation techniques

1 hour 1 minute

Multiple techniques for obturation have been described in literature. This video lecture focuses on those which are widely used literature supports their usefulness. Warm condensation, vertical condensation, core techniques, thermoplasticized gutta-percha, warm vertical compaction, cold lateral condensation and the most recent Sealer-based obturation techniques have all been covered with embedded videos used for demonstration and easy understanding. The armamentarium required for these techniques has also been covered.

Last, but not the least, the important aspect of Orifice sealing and post-endo restoration has been discussed.

Overview

The Endodontic root canal is not just a straight channel from the orifice to the Apex but is a "Complex System" comprising of multiple canals that are interconnected by an isthmus and send out lots of minor branches as laterals, accessories, delta and so on. Hence it is paramount to develop and implement strategies to  "Seal" them off completely following a satisfactory cleaning and disinfection.

3-dimensional (3D) sealing is only as important as 3D cleaning for a long-term prognosis for the case. Where newer strategies to thoroughly clean the Root Canal System are being constantly developed; 3D obturation is no new concept the preliminary demonstration of the endodontic space as a complex 3D entity was made over a century ago -the great Prof. Shilder himself laid down the foundation.

But in contemporary times it appears to be grossly side-lined and ignored and projected as a very complex procedure; which is actually untrue.

This series of lectures will aim to surface the facts related to effectively Clean and Shape the Root Canal System and then thoroughly fill it with the thermoplastic gutta-percha technique (considered the gold standard) in the most simplified manner and with evidence. Also, we would focus on the most recent Sealer-based obturation techniques. You will get to know about various Warm Obturation and Cold Obturation techniques.

The session will certainly change your concept regarding 3D Cleaning & Obturation.
